Before your massage or facial treatment at Zen Body & Skin in Henderson, NV please fill out the following questionnaire. You don’t have to answer all the questions if you are not receiving that particular service. (Although if you are interested in other services in the future please do so.) 

You are also not obligated to fill out the lifestyle questions, however this does help me plan a better treatment protocol. It may also help me pinpoint what may be keeping you from reaching your wellness goals.
